What an Epidural Injection for Sciatica Does

The procedure itself is often brief. Some patients feel temporary pressure or a reproduction of their familiar leg symptoms as medication is placed near the irritated nerve. You will usually need someone to drive you home, especially if sedation is used. Your care team will provide specific instructions for returning to work, exercise, and regular medications.
How Long Does Relief Last?
Results vary. Some people notice relief from the anesthetic on the day of the procedure, though that early improvement can wear off before the steroid begins working. Steroid benefit, when it occurs, often develops over several days and may continue improving for up to two weeks.
For some patients, relief lasts weeks or months. For others, it is modest or short-lived. The most useful result is not always complete pain elimination. A meaningful improvement may be the ability to sleep, stand long enough to prepare meals, return to a walking routine, or make progress in physical therapy without constant leg pain.
If the first injection provides no benefit, repeating the exact same treatment without reconsidering the diagnosis is rarely the right next step. A limited series of injections may be appropriate in some cases, but repeated injections are not a long-term answer for unresolved nerve compression.
Benefits, Limits, and Potential Risks
The main benefit of an epidural injection is that it may relieve inflammation-driven leg pain without surgery. It can be an effective bridge through a painful flare and may help some patients avoid or delay an operation while the body heals. It can also provide diagnostic insight when the medication is placed near a suspected nerve root.
The trade-off is that response is not guaranteed, and the injection does not remove the underlying structural issue. If a large disc herniation is causing progressive weakness, or if severe stenosis continues to limit walking and independence despite well-directed conservative care, surgery may eventually be the more durable option. Surgery is the last option, not the first, but avoiding it at all costs is not the goal. The goal is the least invasive effective treatment for your specific condition.
Most people tolerate epidural injections well. Temporary soreness at the injection site, a short-term increase in pain, facial flushing, headache, or sleep disruption can occur. Steroids can temporarily raise blood sugar, which matters for patients with diabetes. Less common but serious complications include bleeding, infection, nerve injury, allergic reaction, and a spinal headache. Your individual risks depend on your medications, medical history, and the procedure being considered.

An injection works best when it is not treated as a stand-alone event. Once pain improves, the focus should shift to protecting that progress. That may mean guided physical therapy, gradual walking, strengthening the muscles that support the spine, adjusting work demands, and learning how to resume activities without repeatedly aggravating the nerve.
It also helps to establish clear checkpoints before the procedure. What level of pain relief would allow you to function better? What activities are currently impossible? How will you know whether therapy is becoming more productive? Specific goals make it easier to judge whether the injection delivered meaningful value.
